Community Intervention
Community intervention is provided with the intent to alleviate or reduce a recipient’s barriers to community integration or independent living or to minimize the risk of hospitalization or other more restrictive living arrangement. In order to bring change into situations and allow the recipient to function more independently, Community Intervention may be conducted with: an agency, an institution, an employer, a landlord, the recipient’s family, with or without the presence of the recipient. Community Intervention is provided on an individual basis only and may be conducted in person or by telephone.
